Access to records (paper - download and complete)
Use this form if you would like to find out what information we hold about you under the Data Protection Act. A fee is payable.
Access to payment accounts registration form (online)
Open Access allows you to view your business or personal payment accounts online and receive your bills by email. Use the registration form to register for this service.

Anti-social behaviour reporting form (online)
More information about anti-social behaviour. If you need immediate assistance, always dial 999 for the police. You should dial 999 when life is threatened or people are injured, or if there is a crime in progress or an immediate police response is necessary. If you are a tenant or leaseholder of a Council property and are experiencing problems with your neighbours, you should contact your Tenancy Services Officer, email: tenant.services@babergh.gov.uk or see the Council Tenant Anti-social behaviour page for specific information for Council Tenants.

Baberghood Watch Warden nomination form (paper - download and complete)
Residents who want to help keep the Babergh district greener, cleaner and safer can do so by signing up to the Baberghood Watch scheme.

Use this form to apply for housing or council tax benefit.
Benefit fraud report form (online)
Report someone who is claiming benefit payments that they know they are not entitled to. All information you supply is confidential and your identity is not automatically submitted with the form. More information about Benefit Fraud.
Bins - additional bin request form (online)
In exceptional circumstances or where a household has seven or more occupants an additional bin may be provided. This will be a Blue Bin to encourage more recycling material to be collected. More information about requesting an additional bin.

Council House Grass cutting and Hedge Trimming scheme application form (online)
For Council House tenants only. You need to satisfy certain criteria in order to qualify for the Grass cutting and Hedge Trimming scheme.
Council House repair satisfaction form (online)
For Council House tenants only. After you have reported a repair you will automatically be sent a repair satisfaction form. Please complete and return it to tell us about the contractor’s performance.

Council Tax discount form (online)

Garden Waste Collection Scheme (paper - download and complete)
Having a brown bin saves endless trips to the local tip, ensures that your garden waste gets composted and helps towards Babergh’s recycling and composting rate. A fee is payable.
Grants for Business workspace growth application forms (paper - download and complete)
Anyone who owns the freehold or has a substantial leasehold interest in a redundant or under-used building located in the Babergh district may apply for a grant. The application must be made before any conversion or subdivision work has been carried out on the project. Other business grants and loans.

Pollution Prevention and Control permit application forms (paper - download and complete)
Under the Pollution Prevention and Control (England and Wales) Regulations 2000 (the "PPC Regulations"), certain "scheduled" industrial activities and businesses require a permit from the regulator before they can lawfully operate. Tenants Alteration application form (online)
For Council House tenants only. You must normally obtain our approval for carrying out alterations and improvements to your home, in writing, before any work commences. More information about when approval is required for tenants alterations.

Travel Vouchers for up to £100 from 1 April each financial year are to help with travel costs if you are disabled or elderly and reside in a remote area.

Voluntary Surrender of Unwanted Vehicle for Disposal form (paper - download and complete)
Babergh offers a voluntary surrender scheme for people living in the Babergh. There is a fee for this service. The scheme is aimed at those who cannot take their car to an Authorised Treatment Facility (ATF) (or where the cost of collection by the ATF is more expensive than the voluntary surrender scheme), those with incomplete vehicles (who don’t qualify for free take-back) and for those who cannot determine the arrangements for their vehicle.
